Reservation Form

  To attend the 2008 Colorado Ski Trip, the following information is needed.  As airline tickets are issued, we will need the correct spelling of your first and last name.  Also if someone is under the age of 18, then age is required as lift ticket packages are age sensitive.  When the form is completed, print it out, and mail it to Gk-Adventures, 31466 Old Cannon, Beverly Hills, Mi, 48025.  Be certain to enclose a deposit check for $100 per person, and address this check to GK-Adventures.  A confirmation will be sent to you by email after we receive your completed reservation form.
